This is how you will need these documents to be:
Passport: Submit your original passport. Its date of issue shouldn’t be more than 10 years than the present date. It should two blank pages and a validity of 3 months from the date you intend to leave the Netherlands.
US Permanent Residency Card: You’ll need to show your legal residence proof. Bring your work permit too, if it’s applicable.
Photographs: You need to submit one passport-like photo. And it should abide by the Dutch visa photograph requirements.
