Keep the following things in mind about these documents:
Passport: The date of issue of your passport shouldn’t be more than 10 years and its validity should be at least three months from the date of your return to the US. It should have two blank sheets facing each other.
Photo: You need to recent passport-sized picture in color.
Schengen visa application form: You can download this form online. Fill it properly and take its print-out. Now sign it. This was you’ll need to submit to the consulate. Keep a photocopy too.

Language form: No English version of this form is available. It’s available in a choice of three languages—German, French, and Dutch. Fill it properly and give your preference or no preference. Sign the form and retain a photocopy.
Proof of being a US permanent resident: Whether it’s your US resident visa o alien registration card, you either have to submit the original or a notarized copy. The validity of these documents should be three months from the date of your return.
Flight reservations: Roundtrip flight tickets are what you need to show. Most people opt for temporary flight tickets and you should do the same. Don’t make actual books till your visa gets approved.

Your travel itinerary : you MUST BOOK your airline ticket but do not purchase until the visa has been approved.
